چکیده

RECENT reports have indicated that anomalous coronary arteries originating from the pulmonary artery may act as a channel for left-to-right shunt.1-4 Thus a large portion of the coronary blood flow may bypass the coronary capillary bed, pass through arterial anastomoses, and enter directly into the pulmonary artery by retrograde flow through the anomalous coronary with resultant myocardial ischemia. In infants suspected of having this anomaly, early death has been ascribed to this mechanism. This has resulted in a strong plea for early ligation of the aberrant coronary artery in an attempt to redistribute the coronary flow throughout the entire capillary bed.' The observation of successful ligation of the aberrant vessel in one case, with prolonged postoperative follow-up, and of sudden death after the same procedure in another, has led to a revised evaluation of this problem and has prompted this report.

Different Presentation of Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery in Adults: Case Reports

Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital cardiac malformation. We report three cases of ALCAPA who survived to adulthood. The first case was a 51-year-old woman who complained of typical chest pain that was diagnosed with ALCAPA using cardiac catheterization and coronary computed tomographic angiography (CTA). The second case was a 30-...

The Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery (ALCAPA): a Case Series and Brief Review

Background Anomalous left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital cardiovascular defect that occurs in approximately 1/300 000 live births or 0.5% of children with congenital heart disease. There are two types of ALCAPA syndrome: the infant type and the adult type. The most infants experience myocardial infarction and congestive heart failure, and approximately 9...

Anomalous left coronary artery originating from the pulmonary artery. Detailed follow-up report of successful ligation of the aberrant vessel in two infants.

Reestablishment of aortic flow to the coronary vasculature is the optimal treatment for anomalous origin of the left coronary artery originating from the pulmonary artery (ALCA). We performed intraoperative transesophageal echocardiography (TEE) in five patients (median age 10 months; range 1.5-36 months) who underwent revascularization of the left coronary artery without coronary mobilization ...

Anomalous origin of left coronary from right pulmonary artery in hypoplastic left heart syndrome.

An infant with hypoplastic left heart syndrome presented for surgical repair at 9 months of age, the ductus having remained open in the presence of a restrictive atrial septal defect. In addition, an anomalous left coronary artery originating from the right pulmonary artery was found. After preliminary blade/balloon atrial septostomy, a successful modified Norwood procedure with concomitant rei...

Clinical Implications of Congenital Absence of Circumflex Coronary Artery

Introduction: Coronary artery anomalies are rare clinical entities reported in 0.6% to 5.6% of diagnostic coronary angiographies. Anomalous origins of coronary arteries from distal segments are rarely reported. Presented herein is a coronary anomaly in which the circumflex artery (CX) originated as a continuation of the posterolateral branch of the right coronary artery (RCA) w...

Congenital Absence of Left Circumflex Coronary Artery

Congenital absence of left circumflex artery is a rare congenital anomaly of the coronary arteries. The prevalence of the anomaly in different studies ranges from 0.6% to 1.3%. Of these, 80% are benign and asymptomatic and 20% are clinically important. We report a 56-year-old man presented with acute resting chest pain who was diagnosed as having acute anterolateral infarction accompanied by el...
